Who this notice covers
This notice applies to protected health information ("PHI") created or received in connection with care arranged through Tirzepatide Medics, including by the clinicians and pharmacies that provide services through our platform, to the extent they are covered by the Health Insurance Portability and Accountability Act ("HIPAA"). Independent clinicians and pharmacies may also give you their own notices.
Our responsibilities
- We are required by law to maintain the privacy and security of your PHI.
- We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in this notice and give you a copy of it.
- We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by letting us know in writing.
How we use and share your information
Treatment. We use and share your PHI with the clinicians and pharmacies involved in your care, for example to evaluate you, issue and fill a prescription, and follow up.
Payment. We use your information to process payment for services and medications.
Health care operations. We use your information to run our services, improve care, train staff, and for quality and safety reviews.
Business associates. We share information with vendors who perform services for us (such as hosting, scheduling and shipping) under written agreements that require them to protect it.
Other uses and disclosures permitted or required by law
We may share your information without your written authorization when permitted or required by law, including to:
- comply with federal, state or local law;
- support public health and safety activities, such as reporting adverse reactions to medications or preventing a serious threat to health or safety;
- respond to health oversight agencies for audits, investigations and inspections;
- respond to court or administrative orders, subpoenas and law enforcement requests as the law allows;
- report suspected abuse, neglect or domestic violence;
- work with a medical examiner, coroner or funeral director;
- address workers' compensation claims and specialized government functions;
- support research when approved under HIPAA's rules.
Uses that require your written permission
We will never sell your information or use it for marketing without your written authorization. Other uses not described in this notice will be made only with your written permission, which you may revoke at any time in writing (except where we have already acted on it).
Your rights
- Get a copy of your health and claims records, including an electronic copy, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
- Ask us to correct health information you think is incorrect or incomplete. We may say no, but we'll tell you why in writing within 60 days.
- Request confidential communications, for example by email or to a different address. We will say yes to all reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service in full out of pocket, you can ask us not to share that information with your health insurer.
- Get a list of those with whom we've shared your information (an "accounting of disclosures") for up to six years before your request, with certain exceptions.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Choose someone to act for you, such as a person with medical power of attorney or a legal guardian.
