Consent to Telehealth Services
This consent explains how telehealth care works through Tirzepatide Medics. Please read it before your consultation. By booking or attending a telehealth visit, you confirm that you have read and understood it and that you agree to receive care this way.

What telehealth is
Telehealth means receiving health care from a licensed clinician using electronic communication, such as a secure video visit, phone call, online questionnaire or secure messaging, instead of an in-person visit. Your clinician may review your medical history, photos, lab results and other information you share in order to evaluate you, make a diagnosis, recommend treatment and, if appropriate, prescribe medication.
Expected benefits
- Access to a licensed clinician from home, without travel or waiting rooms.
- Faster scheduling and follow-up.
- Ongoing support from your care team through secure messaging.
Possible risks and limits
- A clinician cannot physically examine you, so some conditions may be missed or may need in-person evaluation.
- Information you provide may be incomplete or inaccurate, which can affect your care.
- Technical problems can delay or interrupt a visit.
- Despite security safeguards, electronic communication carries a small risk of unauthorized access.
Your clinician may decide that telehealth is not right for you and recommend in-person care, lab tests or another provider.
Your responsibilities
- Give complete, accurate and current information about your identity, location, health history, allergies and medications.
- Be physically located in a state where your clinician is licensed during each visit.
- Follow your clinician's instructions and tell your care team promptly about side effects or changes in your health.
- Keep your primary care provider informed about your treatment when appropriate.
Prescriptions
A prescription is issued only if your clinician decides it is safe and appropriate. Booking a visit does not guarantee a prescription. Prescriptions are filled by licensed U.S. pharmacies. Compounded medications are not FDA-approved and have not been reviewed by the FDA for safety, effectiveness or quality.
Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
Privacy of your information
Your health information is protected as described in our HIPAA Notice of Privacy Practices and Privacy Policy. Visits are not recorded without your permission.
Your right to withdraw
You may withdraw this consent at any time by contacting us at support@tirzepatidemedics.org. Withdrawing consent does not affect care you already received and does not take away your right to seek care elsewhere.
