Compounded Semaglutide
Semaglutide is a GLP-1 medication for weight management. Compounded semaglutide is prepared by a licensed pharmacy for an individual patient. Here is how it compares with the brands and how we keep your care safe.
The basics
What is compounded semaglutide?
Semaglutide activates the GLP-1 receptor to reduce appetite and slow stomach emptying. The FDA-approved brands are Wegovy® (chronic weight management) and Ozempic® and Rybelsus® (type 2 diabetes), made by Novo Nordisk. In the STEP 1 trial, adults taking semaglutide 2.4 mg lost an average of 14.9% of their body weight over 68 weeks.
Compounded semaglutide is prepared by a licensed pharmacy on a physician’s prescription for an individual patient. Compounded medications are not FDA-approved and are not reviewed by the FDA for safety, effectiveness or quality.

Regulation
Where compounded semaglutide stands with the FDA
The FDA announced on February 21, 2025 that the semaglutide shortage was resolved. Its temporary flexibility for compounding ended on April 22, 2025 for state-licensed (503A) pharmacies and on May 22, 2025 for outsourcing facilities (503B).
503A pharmacies may still compound semaglutide for an individual patient on a valid prescription, but not as "essentially a copy" of an approved product. On April 30, 2026, the FDA also proposed not to include semaglutide on the list of bulk substances that 503B outsourcing facilities may use. Your physician will explain the options available to you, including brand-name medication.
Dosing
How semaglutide dosing works
Injectable semaglutide for weight management is typically started at 0.25 mg once weekly and increased every 4 weeks (0.5 mg, 1 mg, 1.7 mg) to a maintenance dose of up to 2.4 mg, as tolerated. Oral semaglutide is taken once daily on an empty stomach with a small sip of water, at least 30 minutes before food, drink or other oral medicines. Always follow your label and your physician’s instructions.

FAQ
Common questions
Is compounded semaglutide the same as Wegovy?
It contains the same active ingredient but is not the same product. Compounded semaglutide is not FDA-approved and was not studied in the STEP trials.
Should I choose semaglutide or tirzepatide?
In the SURMOUNT-5 trial, tirzepatide led to more weight loss on average (20.2% vs 13.7%). Semaglutide may suit people who prefer an oral option or have tolerated it well before. Your physician helps you decide.
Can I switch from semaglutide to tirzepatide?
Often, yes, under a physician’s guidance. Don’t take two GLP-1 medications at the same time.
